To verify your chiropractic coverage, please email a photo of the front and back of your insurance ID card, along with the subscriber's date of birth, to info@proreliefchiropractic.com.

We will verify your benefits and explain them to you at no charge prior to your treatment. Please keep in mind that you will likely need to meet a deductible before your co-pay or co-insurance applies. Additionally, chiropractic visits are typically categorized as specialist appointments, which may result in a different fee than a visit to your primary care physician.

Please note that some Cigna insurance plans cover chiropractic care as an out-of-network benefit.

UnitedHealthcare (UHC) and UMR, please be advised of the following outpatient costs:

- New Patient Visit: Copay amount  + $50

- Follow-up Visits: Copay amount + $25

The additional fees cover specialized services, including Dry Needling and Active Release Technique.

 

*When using an in-network provider, “non-covered” charges such as Active Release Technique and Dry needling will still incur out-of-pocket costs. 

 

* Please be advised that dry needling is not covered by insurance. As a result, an additional $25 fee will be collected at the time of service.
